Older Families Planning Project
The Older Families Planning Project (OFPP) is a partnership between Sheffield City Council, Sheffield Health & Social Care (SHSC) NHS Foundation Trust and the Sharing Caring Project. It helps to support the oldest and most isolated families to get the support they need now, and to plan for the future with greater confidence.
The OFPP team includes a social worker and development worker based in the Council, an occupational therapist and community nurse based in SHSC Trust and a person centred planning facilitator, administrator and coordinator all based at the Sharing Caring Project and employed by Sheffield Mencap.
The Project helps older families to think through the different emergency and long term optins available and helps develop plans that are realistic and achievable. Work has included helping people to stay on in their own home following the loss of their elderly carer, helping people make planned moves to alternative accommodation, usually supported living placements, and helping people to develop the skills and confidence that may be needed now and for the future. Everything the project does is underpinned by person centred planning approaches and the overall aim is to avoid crisis and give famlies greater confidence and peace of mind for the future.
